Ready to book your DOT exam?
A Department of Transportation (DOT) physical is a medical screening that commercial motor vehicle (CMV) drivers are required to complete and pass to maintain a valid commercial driver’s license (CDL).
According to the Federal Motor Carrier Safety Administration (FMCSA), if you are a CMV driver, you must undergo a DOT physical conducted by a certified medical examiner and receive a medical examiner’s certificate (MEC) – also called a DOT health card. Without a valid DOT health card, you can lose your CDL. A certified medical examiner will issue you a DOT health card after you meet the DOT physical requirements.
DOT physicals are highly regulated for the drivers’ safety and are designed to detect physical, mental, and emotional issues that can affect a driver’s ability to safely drive a commercial vehicle. Our physicals are completed by certified medical examiners, specially trained to understand the regulations and prevent drivers from being inappropriately disqualified and ensure the safety of the driver and the public.

What is the cost of the DOT exam? The cost is $125 and is not covered by insurance
How do I book an exam? To book your exam you must do so online. Then, please complete the general consent and our staff will call you to confirm your appointment.
What if I need to cancel my appointment? You may cancel anytime by calling our office at 708-607-2503, or from the email or text message that you will receive after booking. There is no fee to cancel. We do charge a no show fee if you do not cancel and do not show up for your appointment.
What do I need to bring to the appointment with me? Please bring your current drivers license/CDL license, all medications bottles, any records from your primary care provider, or specialist, any recent labs, or imaging, and any exemption forms. You may also fax these over to us at 778-200-3824 or email info@physicalsplus.net prior to your appointment. You can also complete any of the appropriate forms below and bring but we will provide at your appointment.
Is there anything else I need to know? Be prepared to provide a urine sample, complete a vision screening and possibly have an A1C test completed. Remember, this is a medical examiners assessment to determine if you are safe on the road so a visit does not guarantee an automatic pass. We may need more information the exam so it is important to disclose all your health conditions and bring any pertinent records to the appointment.
