Physical exams alone are not covered by insurance. Please bring any forms, prior medical records, medication bottles with you. For school physicals please bring current vaccine records Please complete the forms that you will receive upon booking in your email. Arrive at least 15 minutes early to complete any paperwork if not completed online.
