What should I bring to my physical exam?

Sports Physical (under 18):

• Parent or legal guardian must accompany the patient

• School ID (if in high school)

• List of current medications

• Any prior injury or relevant medical records

School Physical:

• Parent or legal guardian if under 18

• School ID (if applicable)

• Vaccination records (required)

• List of medications

• Any medical records if applicable

Work Physical:

• Required employer forms that need to be completed

• List of medications

• Any medical records or specialist documentation (if applicable)

What is the cost of a physical exam?

• Work physical: $60

• School physical: $30

• Sports physical: $30

• School + Sports physical (same visit): +$25 additional form fee
  All physicals are self-pay and not covered by insurance.

How do I book an appointment?

All appointments must be booked online. 

​

What is your cancellation policy?

You may cancel anytime by phone or through your confirmation message. There is no cancellation fee if you notify us in advance. A no-show fee may apply for missed appointments.

Do I need a TB test?

If required, a TB test can be added to your visit.

• TB placement: $20

• TB reading: Free (48–72 hours later visit required)

Failure to return within the required window will require a repeat test and additional charge.

Are there additional costs?

• Each additional form: $25

• TB testing and lab work: additional fees apply

• Drug testing or specialty physicals: starting at $150+, depending on requirements

What else should I know before my visit?

Please bring all required documents and disclose any medical conditions during your exam. Additional information may be needed after your visit, and completion is not a guaranteed clearance.

Work, school, and sports physicals do not replace an annual wellness exam.
