Dear Physicals Plus Health & Wellness community,
As an essential business we will continue to remain open during this time. However, to continue to ensure the health and safety of all our patients and staff here is what we have been doing and other changes that we have implemented:
•Visits continue to be by appointment only
•COVID screening to be completed prior to every appointment
•We still require a mask to be worn over the nose and mouth while in the building at all times
•We are asking that payments be made by placing a card in your file or by quick pay (Zelle) to firstname.lastname@example.org for touchless check out
• If you are a new patient, please read and sign the consent electronically.
•We do not currently test for COVID in our office, if you are having symptoms you may complete a virtual visit with our provider prescriptions can be sent to your local pharmacy.
